This page contains two documents that govern your use of smiledesignbykc.com (the "Site"): our Privacy Policy, which explains what information we collect and how we use it, and our Terms of Service, which set the rules for using the Site. By using the Site, you agree to both.

The Site is operated by Kirti DDS PC d/b/a Smile Design Studio by KC, the dental practice of Kirti Chopra, DDS, located at 255 S. 17th Street, Suite 2507, Philadelphia, PA 19103.

Three things to know first

This Site is informational, not dental advice. Reading it, submitting a form, or requesting an appointment does not create a dentist-patient relationship. That relationship begins only when you are seen and accepted as a patient of the practice.

Do not use this Site for an emergency. If you are experiencing a medical emergency, severe swelling, difficulty breathing or swallowing, uncontrolled bleeding, or trauma, call 911 or go to the nearest emergency room. For an urgent dental problem during business hours, call our office at 215-545-4024. Website forms and email are not monitored continuously.

Do not send health details through this website. Please keep treatment history, symptoms, insurance identification numbers, and other sensitive information out of our website forms and out of ordinary email. Those channels are convenient but not secure. To discuss clinical matters, call the office or use the patient portal linked from the Site.

Part One: Privacy Policy

About this policy and your health records

This policy covers information collected through our website. It is not our Notice of Privacy Practices, and it does not govern the protected health information in your dental record. Patient health information is handled under separate policies required by federal and Pennsylvania law, and our Notice of Privacy Practices is available at our office on request.

Information you give us

You can browse the Site without telling us who you are. We collect personal information only when you choose to provide it, including when you:

  • Submit a contact form, appointment request form, or offer request form. These forms typically ask for your name, email address, phone number, and any message you write. Our forms are processed by a third-party form service, which delivers the submission to us by email.
  • Request an appointment through our online scheduling link. Online booking is handled through our patient portal, which is operated by our practice management software provider on its own website. When you follow that link, you leave our Site and your information is collected by that provider under its terms and privacy practices, along with our obligations as a health care provider.
  • Call the phone number listed on the Site. Calls to our office are answered by our staff and handled under our office policies. Some calls to our office are recorded for quality and training purposes; when call recording applies, you will hear a notice at the start of the call, consistent with Pennsylvania law.
  • Email us, respond to a promotional offer, or otherwise contact us directly.

Information collected automatically

Like most websites, the Site automatically collects limited technical information when you visit, including your IP address, browser type and version, device type, operating system, the pages you view, the links you click, the time and date of your visit, and the website or advertisement that referred you.

We use the following services to collect and process this information:

  • Google Analytics 4 — measures how visitors find and use the Site so we can improve it. Google Analytics sets cookies and collects usage data. Information is generally reported to us in aggregate.
  • Google Search Console — reports how our pages appear in Google search results. It does not identify individual visitors to us.
  • Google Ads conversion and remarketing tags — measure the performance of our advertising and allow our ads to be shown to people who have previously visited the Site. These tags may set cookies or similar identifiers in your browser.

We do not use this information to identify you personally, and we do not combine it with your dental record.

Advertising and remarketing

We advertise our practice online, including through Google Ads. We use remarketing, which means that after you visit our Site you may later see our ads on other websites and apps.

Third-party vendors, including Google, show our ads on sites across the internet. These vendors use cookies and device identifiers — including Google's own cookies and, where applicable, first-party cookies such as the Google Analytics cookie together with third-party cookies — to serve ads based on your previous visits to our Site. We do not receive your name or contact details through this advertising, and our ads are never targeted based on your dental records or any health information you have given us as a patient.

Opting out means you may still see our ads, but they will not be based on your prior visits to our Site.

We use the information described above to:

  • Respond to your questions, requests, and appointment inquiries
  • Schedule, confirm, and follow up on appointments
  • Provide dental care and administer our practice, including billing and insurance
  • Send information you have asked for, including details about promotional offers

  • Measure and improve our marketing and advertising
  • Comply with legal, regulatory, and professional obligations

We do not sell your personal information, and we do not rent or trade it for money or for other companies' advertising purposes.

We work with vendors who help us operate — including our website host, our form processing service, our practice management and patient portal provider, our information technology and compliance vendor, our marketing agency, and our analytics and advertising providers. They may access information only as needed to perform services for us. Vendors who handle protected health information do so under written agreements required by federal law.
  • Professional and legal requirements. We may disclose information when required by law, court order, subpoena, or regulatory authority, or to protect the rights, property, or safety of our patients, our practice, or others.
  • Business changes. If our practice is sold, merged, or reorganized, patient and business records may transfer as part of that transaction, subject to applicable law.

Third-party links and services

The Site links to services we do not control, including our online scheduling portal, Google, and our social media profiles. When you follow those links, the operator of that service collects your information under its own privacy policy. We provide those links for convenience, and a link does not imply endorsement. We encourage you to read the policies of any site you visit.

Security

We use reasonable administrative, technical, and physical safeguards to protect the information we hold, and our practice systems are supported by a vendor engaged for information security and compliance. No website or method of transmission over the internet is completely secure, and we cannot guarantee absolute security of information transmitted to or from the Site.

Retention

We keep website inquiry information for as long as needed to respond to you and for our ordinary business records. Patient dental records are retained for the period required by Pennsylvania law and professional standards, which is separate from this Site.

Children

The Site is intended for a general adult audience and is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through the Site. Appointments for minor patients should be requested by a parent or legal guardian. If you believe a child has provided us information through the Site, contact us and we will delete it.

Your choices

  • Email. You can unsubscribe from practice emails using the link in any message, or by contacting us. We may still send you messages about your care and appointments.
  • Access and deletion. You may ask us what website inquiry information we hold about you and ask us to correct or delete it. Requests about your dental record are handled under our health information policies rather than this one.
  • State privacy rights. Some states give residents additional rights over their personal information. If you believe you have such rights, contact us and we will respond as required by applicable law. Health information held by a health care provider is generally covered by federal health privacy law rather than state consumer privacy laws.

Part Two: Terms of Service

These Terms of Service ("Terms") govern your use of the Site. By accessing or using the Site, you agree to them. If you do not agree, please do not use the Site.

The content on this Site — including descriptions of services, treatment information, blog articles, and photographs — is provided for general information only. It is not dental or medical advice, and it is not a substitute for an examination, diagnosis, and treatment plan from a licensed dentist who has evaluated you in person.

Using the Site, reading its content, submitting a form, or requesting an appointment does not create a dentist-patient relationship. That relationship begins only when you are seen and accepted as a patient of the practice.

Never disregard or delay seeking professional advice because of something you read on this Site.

Appointment requests

Forms and scheduling links on the Site let you request an appointment. A request is not a confirmed appointment. An appointment is scheduled only when our office confirms the date and time with you directly.

Online scheduling is provided through our patient portal, which is hosted by our practice management software provider on its own site and subject to that provider's terms.

Photographs and treatment results

Before-and-after photographs on this Site are of actual patients of this practice, used with permission, and are not models or stock images. Images may be cropped or have identifying features obscured to protect patient privacy; they are not altered to change the clinical result shown.

‍

Every patient is different. Dental results depend on individual anatomy, oral health, bite, home care, and other factors. The results shown on this Site are examples of what was achieved for those patients and are not a prediction, promise, or guarantee of the result any other patient will obtain.

Fees, promotions, and insurance

Any fee, fee range, or promotional offer described on this Site applies only as stated on the page where it appears and only to the services listed there. Treatment fees depend on the diagnosis and treatment plan developed for you after an examination, and the actual fee for your care may differ from any figure shown on this Site.

Promotional offers may be changed, extended, or discontinued at any time. Descriptions of insurance participation, membership plan benefits, and payment or financing options are general summaries; the terms of your specific plan, benefits, and eligibility control what is actually covered. Third-party financing is offered by outside lenders under their own applications, terms, and approval criteria, and we do not control their decisions.

Intellectual property

The Site and its contents — text, graphics, logos, photographs, video, page design, and the marks "Smile Design Studio by KC" and associated designs — are owned by Kirti DDS PC or its licensors and are protected by copyright, trademark, and other laws.

You may view, print, and download content for your own personal, non-commercial use. You may not copy, republish, distribute, modify, frame, scrape, or use any content or images for any other purpose without our prior written permission. Patient photographs may not be reproduced or reused under any circumstances.

If you send us a message, review, testimonial, photograph, or other content, you confirm that it is yours to send and that it does not violate anyone else's rights. You give us permission to use it for the purpose you sent it. If we would use a testimonial, review, image, or likeness in our marketing, we will obtain your separate written permission first.

To the fullest extent permitted by law, Kirti DDS PC, Smile Design Studio by KC, Kirti Chopra, DDS, and our employees and contractors will not be liable for any indirect, incidental, special, consequential, or punitive damages, or any loss of data, revenue, or profits, arising out of your use of or inability to use the Site — even if we have been advised of the possibility of such damages.

Nothing in these Terms limits any liability that cannot be limited under applicable law, and nothing in these Terms applies to or limits our professional obligations to our patients, which are governed by law and professional standards rather than by this Site.

Indemnification

You agree to indemnify and hold harmless Kirti DDS PC and its owner, employees, and contractors from any claims, damages, losses, and reasonable expenses arising from your misuse of the Site, your violation of these Terms, or your violation of the rights of another.

Governing law

These Terms are governed by the laws of the Commonwealth of Pennsylvania, without regard to its conflict of laws rules. Any dispute relating to the Site or these Terms will be brought in the state or federal courts located in Philadelphia County, Pennsylvania, and you consent to the jurisdiction of those courts.

Severability

If any part of these Terms is found unenforceable, the rest remains in effect.

Changes to this page

We may update this Privacy Policy and these Terms of Service from time to time. The effective date at the top of this page shows when they were last revised, and the current version is always posted here. Continuing to use the Site after a change means you accept the update.
